- Description: No prescription, no doctor, no appointment, and a third of the price. Thailand's pharmacies are genuinely useful and genuinely a place to be prescribed wrong.

You have a urinary tract infection, and what you want is a prescription and a short conversation with somebody qualified. What you get is a counter, a person behind it, a question about your symptoms, and thirty seconds later a box of antibiotics handed across without anybody having established that is what is wrong with you. It is also genuinely cheap, and in a country where a [clinic](https://easythailandvisa.com/thailand-hospitals-and-clinics-for-tourists/) visit costs more than your flight home, that trade is one you will probably make anyway.

## How it actually works

**Thailand has thousands of licensed retail pharmacies, and a large number of them will sell you almost anything you ask for without a prescription.** This includes antibiotics, which is a notable difference from most countries and a genuine public health problem, and also the reason the rules on controlled substances are stricter than you might expect.

**There are two tiers.** A registered pharmacist is behind the counter in the licensed stores, typically the ones with a prominent green cross. Many smaller shops are retail drugstores with a person on the till who has no pharmacological training whatsoever.

**The staff will ask you what is wrong,** and that question is the entire safety mechanism. Where the person behind the counter is a pharmacist, it is a real consultation and can be genuinely useful. Where it is not, it is a sales conversation, and the second conversation you have with the shop next door will be with a different person who will be happy to suggest something else.

**Prices are a fraction of Western ones,** and generics of almost everything are available. That is the reason a lot of people do their serious medication shopping here.

## What you can usually get

**Over-the-counter basics:** paracetamol, ibuprofen, antihistamines, antacids, oral rehydration salts, and topical antibiotic creams.

**Prescription antibiotics,** commonly and without much argument.

**Anything you bring a valid prescription for from home,** which is a well-run and legal route, and the right one.

**And some things you should absolutely not buy here,** which is the next section.

## What you must not buy, and why

**Controlled and psychoactive drugs are a different matter entirely,** and unlike the antibiotics, attempting to obtain them is a criminal offence in Thailand with real penalties. This is covered in detail in the drug penalties post and it is not something to test with a friendly pharmacist.

**Do not buy an opioid or a benzodiazepine without a prescription, and do not bring in a supply of one.** The rules on your own medication and the rules on someone else's prescription are both enforced, and the penalties are measured in years. This is the one area where the relaxed counter behaviour does not extend and you should assume nothing is permitted.

**Do not buy an antibiotic for something that is not an infection.** This is the most common way a Thai pharmacy causes harm. A sore throat in a traveller is viral a large proportion of the time, and a course of antibiotics for it does nothing, costs money, and contributes to resistance.

## Where to go instead

**For anything that could be serious, go to a clinic, not a pharmacy.** International clinics in Bangkok and the tourist cities have English-speaking doctors, proper diagnostics, and they will send you to a hospital if it is needed. The cost is a fraction of a Western consultation and the fraction of an American one is large.

**For the specific things a pharmacy cannot safely help with, here is the list:**

**Chest pain, difficulty breathing, severe abdominal pain, a head injury, a serious burn, anything involving loss of consciousness, or any injury where a bone might be broken.** A clinic is an hour away and a hospital is better.

**Anything that has been getting worse for more than a few days.** Traveller's diarrhoea that has not settled, a fever that has persisted, a wound that is not healing.

**Anything involving a bite, a sting or a wound you did not see inflicted,** because [rabies](https://easythailandvisa.com/thailand-temple-monkeys-rabies-what-to-do/) exists here and the post-exposure treatment timeline is the entire reason this matters.

## Practicalities

**Bring the packet or a photo of it** if you are replacing something, because the brand name matters and the generic may not be the thing you had before.

**Ask about dosage rather than accepting a packet,** and ask whether it is something you can take alongside whatever else you are taking.

**And keep the box, or a photograph of it,** for a border or an insurance claim. This is not paranoia, it is the single most common thing that slows down a police report or a medical bill.

## The honest summary

The pharmacy is excellent for the small, boring, predictable complaints that make up most of what goes wrong on a trip. It is not a substitute for a doctor, and the specific category where the gap between a helpful shopkeeper and a qualified pharmacist is widest is antibiotics, which is also the category people most often self-medicate with abroad.

Know which is which before you need it, and know the list above well enough to not stop and think about it at midnight.
